This form is used to verify your eligibility to receive your program degree/diploma and should be completed at the beginning of your final semester.
If graduation requirements are not met, a new form must be submitted each time you register for the last course(s) in your program(s).
December Graduates:
Fill out the form below to receive your diploma/degree in the mail. Your degree will not be processed until this form is received.
If you plan to participate in the commencement ceremony in May (open to both December and May Associate Degree and Technical Diploma graduates) you must submit the graduation candidacy form no later than April 25th (March 15th to be in the program) – no exceptions.
