MPTC uses a product called Dynamic Forms for electronic forms. Forms that need a signature, require a Dynamic Forms account in order to complete the form. (With this process, Dynamic Forms is authenticating that the person creating the account is the owner of that e-mail address).
Creating a New Dynamic Forms Account
Steps to create a Dynamic Forms account:
- Visit https://dynamicforms.ngwebsolutions.com and click the “Create a New Account” link.
- Follow the prompts to create a username and password. (Take note of your new Dynamic Forms username and password — you’ll need both to log back into your Dynamic Forms account to complete your form.)
- Check your personal email for the verification link. Please lookout for an email from notify@ngwebsolutions.com (Hint: Check your email on the same device you registered on. If you don’t see the verification email, look in your junk folder.)
- Click the verification link in the email and follow the prompts.
- When you arrive at the Dynamic Forms Dashboard, you have successfully completed the account creation! Close this tab.
- Go back and access the link to the form that you need to complete. You will be prompted to sign into your newly created Dynamic Forms account. Once signed in, you will be presented with the form to complete, sign and submit.
