  • These requirements are set by the clinical agencies which require compliance with their employee health standards.
  • Failure to submit ALL required documentation by the due date will result in your ineligibility to participate in clinical experiences.
  • The above chart outlines current health requirements for all MPTC health students. Should you be assigned to a clinical agency with additional requirements this will be communicated, in advance, to you and you will be expected to comply within the provided timeline in order to continue in your program progression.
  • All fees associated with health requirements are the student’s responsibility.
  • Acceptable documentation:
  1. Vaccines: Wisconsin Immunization Registry Report (https://www.dhfswir.org), Medical Record (e.g. MyChart), Positive Titer test results
  2. TB: Proof of 1 TB blood test or 2 TB skin tests within the last 12 months
  3. Physical Exam – within last 12 months, document signed by provider
  4. CPR: Certificate of completion of an American Heart Association BLS class
  5. N95 Fit Test: Fit Test record from the clinic
  6. Drug test: Must be done at specified labs, do not do this with your primary care provider. Results are sent directly to MPTC from the lab/clinic. Must be done within 30 days of course start.
