Please submit the following form if you are a Moraine Park student or employee and would like to request a seat on the bus shuttle to the Health & Wellness Fair in Fond du Lac. If you are an employee, please coordinate a visit to the fair with your supervisor.
Important: Shuttle seating is first-come, first-served and currently limited to 15 passengers. Please only reserve a seat if you plan to attend and do not have alternative transportation to the fair. Shuttles will depart promptly at their scheduled times and cannot be held.
