You will need to create a Dynamic Forms account to authenticate into the form. Once you click on the link above, you will start with creating your Dynamic Forms account. Following are a few helpful hints on the process for creating your Dynamic Forms account:

• Use a personal email (Yahoo, Gmail, Outlook, etc.) when creating your account. Take note of your username and password — you’ll need both to access your form.

• Dynamic Forms will send a verification email to the address you provided, with the subject line “Activate your account.” After clicking on the verification email, your Dynamic Forms account will be set up.Helpful hint: Check your email on the same device you registered on. If you don’t see the verification email, look in your junk folder.
• The verification email from Dynamic Forms includes a link to continue to the form. Follow this link and use your verified username and password to log in and access the MPTC form. That’s it!
