Complete this form and attach an unofficial copy of your college transcripts to request permission to register for an MPTC undergraduate course.*
*Permission is not required to register continuing education courses. Follow the link below for information on registering the Nursing Assistant course: https://www.morainepark.edu/academics/programs/nursing-assistant/

If you do not have college transcripts, please submit a document showing your ACT, SAT, or Accuplacer Placement scores.

Once your request has been reviewed, you will receive an update via email indicating the results.
