Educational Records

A record of student activity (handwritten, print, tapes, film, computer or other medium) is maintained by the Records Office at the Fond du Lac campus. Current students are encouraged to review their unofficial record via the Student Information System. Copies of or inspection of educational records is available via written request to the Registrar’s office. Students can also request an official transcript.

Moraine Park Official Transcript Requests

An official transcript can be requested in writing and remittance of a $6.00 processing fee at any campus location.  Request forms must include a signature and are available online (click here) or at any student service campus location. Immediate transcripts can be provided in person at the Fond du Lac campus location only, for a $10.00 processing fee.

Unofficial Transcript Requests

Current students are encouraged to print their unofficial transcripts via the Student Information System.

Please email your request to transcripts@morainepark.edu. Unofficial transcript can be mailed or picked up in person at the Fond du Lac campus.

Please provide your name, previous names, student ID number (if known) and current mailing address information in this request.

Enrollment Verification

Verification of enrollment can be provided to banks, employers and other agencies. Please submit an Authorization to Release form, agency form or written request with specific details (mailing address, fax number) of the information you want released to verify your enrollment.

GED/HSED Records Requests

MPTC GED/HSED transcripts are processed only at the Fond du Lac Campus. A signed record request form is necessary to process all requests. The form is available at all campuses or by clicking here for the PDF version.

High School and/or Post-Secondary Transcript Submission

Students may need to submit high school and/or post-secondary transcripts for admission and credit for prior learning evaluation to Moraine Park Technical College. Please contact the high school or post-secondary institution you attended for information about requesting an official transcript and fee details.
