Transcript Instructions and Request Form

High school and post-secondary (college) transcripts are needed to complete Admission to Moraine Park Technical College. The form below may be used to request an official transcript from the high school and/or college(s) you previously attended.

Print and submit one form for each institution that you can request transcripts from. Institutions normally charge a fee for transcripts, which must accompany your request. It is strongly recommended that you contact the institution for this cost prior to sending your request. Institutions will not send transcripts unless this fee is included.
